Corrections

We publish public records as they appear in the original source. If something is wrong, we want to fix it quickly.

What we can correct

  • Matching errors — a penalty, action or rating attached to the wrong agency. We fix these as soon as we confirm them.
  • Extraction errors — a date, amount or name that doesn't match the state or Medicare document.
  • Outdated records — a penalty reduced or overturned on appeal, a status that has changed, or contact details that are out of date. Please include documentation (for example, an HHSC letter or order).

What we can't change

  • Accurate public records, even if an agency disagrees with the original finding. If HHSC or Medicare changes its record, we'll update ours on the next data refresh.
  • Scores, except as the automatic result of corrected data. No one can pay for, or negotiate, a different score.

How it works

  1. Send the form below with the agency, what's wrong, and a link or document showing the correct information.
  2. We check it against the original public source.
  3. If we confirm an error, we fix it and note the date of the correction. If not, we'll explain why.

Agency staff: please don't include any patient information in your request.
